If you are a natural person in the United States who used the LocalMed Portal to schedule, modify, or re-schedule an appointment between March 2022 and June 2025 and you shared information that was allegedly disclosed or transmitted to a third party by Dental Intelligence, Inc. (“Defendant”), you may be a member of the class and entitled to money. Excluded from the Settlement Class are: (i) the Released Parties; and (ii) the Court, Court personnel, and members of their immediate families.
If you are a Settlement Class Member and wish to receive a cash payment then you must submit a Claim Form by January 25, 2027.
To start your online claim form, you must login with your Login ID and PIN. You can find your Login ID and PIN on the top of the Notice that was sent to you. If you are unable to locate your ID and PIN, please contact us by emailing info@DISettlement.com. Please request your Login ID and PIN by providing your full name and mailing address.

The deadline to submit a Claim Form online is January 25, 2027. If you are mailing your Claim Form, it must be mailed with a postmark date no later than January 25, 2027.
